Birth Records		To obtain a birth record, you must give the number of copies, the full name of the child as it appears on the birth record, the date of birth, the place of birth, the father's full name, and the mother's maiden name. To obtain a copy of the birth record for an ancestor, heirs must give the date and place of death and provide additional documentation, such as a certified copy of the death certificate (or photograph of the tombstone). You may be asked to show your genealogical chart. You must also present a driver's license or state issued id. If requesting by mail, you must enclose a clear copy (front and back) of your current driver's license or state issued id, your signature, a return address, and a daytime phone number. Birth records for adopted children and for anyone born prior to 30 October 1978 whose parents were not married at the time of the child's birth can only be obtained from the Michigan Department of Community Health in Lansing. Forms are available at the Washtenaw County Clerk/Register's office or by calling Community Health at 517-335-8656.
Death Records		To request a death record, you must give the number of copies, the full name of the deceased person as it appears on the record, the date of death, and the place of death. If requesting by mail, include your signature, return address, and daytime phone number. Indexes and records from 1970 to the present are searched only by staff with a written request and payment of fees.
Marriage Records		To request a marriage record, you must give the number of copies, the full maiden name of the bride and the full name of the groom as they appear on the record, the date of marriage, and the place of marriage. If requesting by mail, include your signature, return address, and daytime phone numer. Indexes and records from 1970 to the present are searched only by staff with a written request and payment of fees.
